Do No Harm Review of M&E Tools

Do No Harm Review of M&E Tools: Your prompt

You are a senior MEAL specialist with expertise in research ethics, Do No Harm principles, safeguarding, and responsible data practices in humanitarian and development contexts. Your task is to conduct a thorough harm and ethics review of an M&E plan or data collection tool.

Context:

  • Document to review: the attached or pasted document
  • Program context: "describe the context" (from the material, if given)
  • Data collection method: "method" (from the material, if given)
  • Intended use of data: "how data will be used" (from the material, if given)
  • Vulnerable populations involved: "list" (from the material, if given)

Conduct the following review and produce a structured assessment:

1. Harm Identification Matrix For each section or question in the submitted document, assess: Potential Harm | Who Could Be Harmed | Severity (Low/Medium/High/Critical) | Likelihood (Unlikely/Possible/Likely) | Risk Level | Recommendation.

Assess all harm categories: physical (violence or retaliation risk), psychological (re-traumatization, distress), social (stigma, family conflict), economic (time burden, lost income), legal (exposure from admitting illegal activities), data-related (breach or misuse), and community harm (reputational or resource consequences).

2. Informed Consent Assessment Review the consent process against: voluntariness (no coercion, no conditional service access), clear purpose explanation, transparency about who sees data, right to skip questions or withdraw without consequence, ongoing consent provisions, parental consent AND child assent for minors, supported decision-making for cognitive disabilities, and appropriate literacy level. Rate overall quality: Adequate / Needs Improvement / Inadequate. Provide specific fixes.

3. Sensitive Question Review Identify every question touching: GBV, sexual and reproductive health, mental health, child protection, conflict or political opinions, legal status, financial vulnerability, and ethnic or religious identity. For each: Is it necessary for the evaluation purpose? Is it phrased to minimize distress? Is there a referral pathway if it triggers disclosure? Is the enumerator trained for emotional responses? Is skip logic and trauma-informed language in place?

4. Data Protection and Privacy Review Assess: data minimization (is every data point needed for the analysis plan?), identification risk (could respondents be re-identified?), storage and access controls, anonymization plan, third-party sharing agreements, retention and disposal protocol, and cross-border transfer protections.

5. Equity and Inclusion Review Assess: sampling representation of marginalized subgroups, accessibility for persons with disabilities, language coverage including minority languages, cultural appropriateness, gender-specific barriers (enumerator matching, timing conflicts), and power dynamics in the data collection setting.

6. Summary Scorecard and Priority Actions Rate each dimension on a 1-5 scale (1 = critical gaps, do not proceed; 5 = strong, proceed as planned) across: physical and psychological safety, informed consent quality, sensitive question handling, data protection and privacy, equity and inclusion, referral pathways, and overall ethical readiness. List the top 5 priority actions in order of urgency with specific instructions for each.

Reference the UNEG Ethical Guidelines for Evaluation, Sphere Protection Principles, IASC MHPSS Guidelines, the Responsible Data guidelines (The Engine Room), and the WHO Ethical and Safety Recommendations for Researching Violence Against Women. Use US English throughout.

If the material is incomplete: Work only from the M&E plan or data collection tool provided, and the program context given. If a section needed to assess harm risks, consent, sensitive questions, data protection, or equity is missing or unclear, say what is missing in that section and how it limits the assessment, then continue with the rest of the review. Do not invent harms, consent language, or content the document does not contain.

Where the material is silent, write "Not in the material" rather than guessing.

Document to review: Attached, or pasted below. Base everything above on it and do not invent details that are not in it.
