Worked Project: Fictional Input

The fictional source pack used throughout the worked project.

Status: Entirely fictional, authored to illustrate the linked evaluation-planning, feasible-measurement, and reporting-to-action workflows. This is not a client file, test output, or practitioner trial.

HP-01 Program and decision

Hillside Kitchen Gardens ran a 5-session demonstration course for adults at two community gardens during January-March 2026. The program director will decide whether to repeat the course format and whether to consider a follow-up visit for the next cohort. No decision date or approval to add a follow-up visit was supplied.

HP-02 Approved completion measure

  • Indicator: Number of enrolled participants completing the course.
  • Definition: An enrolled participant is a completer after attending at least 4 of the 5 sessions.
  • Population and period: 40 enrolled adults, January-March 2026.
  • Target: At least 32 completers by March 31, 2026 (80% of 40).
  • Source: Session attendance register with 40 participant IDs and five session columns. The definition and target are approved for this cohort.

HP-03 Attendance summary

The register shows 31 of 40 participants attended at least 4 sessions. No prior-cohort comparison or earlier Q1 result is included. No discrepancy review has yet been recorded.

HP-04 Participant feedback

After the final session, 24 participants were invited to answer: “Did you find the garden-practice demonstration useful?” Eighteen returned a form: 14 answered Yes and 4 answered No. Six invited participants did not return a form. The selection reason for inviting these 24 rather than all 40 was not recorded. No question measured later use of the practices.

HP-05 Collection and staff capacity

Two facilitators maintain attendance, one per garden. Each spends about 15 minutes per week on M&E attendance records during each of the 12 reporting weeks in the January-March quarter. Each has up to 30 minutes per week available for M&E records.

The monitoring officer consolidates both garden logs once per month, taking about 20 minutes. In March, the officer also spends 45 minutes reconciling and preparing the quarterly report. The officer has up to 90 minutes per month for this cohort's M&E work. These are the supplied planning estimates. They do not include a new follow-up visit or survey.

Data flow: Facilitators enter attendance in garden registers. The monitoring officer consolidates the registers and prepares a monthly summary and quarterly report for the program director.

HP-06 Draft report text

“Course completion is increasing steadily and is on track. Participants find the demonstrations useful, so we should add follow-up visits next cohort.”

The supplied records contain only this one cohort and one reporting period. The target is an end-of-period target, not a time-phased trajectory. No follow-up use data, comparison group, or causal evaluation design is supplied.

HP-07 Access and unresolved decisions

The attendance register, monthly summary, quarterly report and feedback forms can be reviewed for this exercise. No permission to contact participants again or conduct site visits was supplied. No operational definition or measure of “later use” was approved. No owner, date, budget or approval for follow-up visits was supplied. There is no prior DQA or action-closure record in this source pack.
