Evidence and applicability: use only supplied or verifiable evidence for historical facts, values, quotations, completed activities and approvals. Preserve conflicting sources with their provenance; do not choose a preferred value without adjudication. Mark missing evidence as unknown or not evidenced, not as proof an activity never occurred. Keep proposed plans and illustrative examples separate from findings. Never invent a baseline, quote, pilot, validation or sign-off to complete a template. Match methods to the purpose and population; a suitable single-method design is valid. Distinguish descriptive change from causal attribution. Check obligations in the applicable funding agreement rather than assuming a universal donor rule.

Extract the findings, quantitative results, qualitative insights, recommendations, and limitations in the document I provide. Ask for the document if absent and use a focus area only if I give one.

Build a traceable evidence register rather than rewriting the whole report. For each relevant finding, capture the exact source location, claim, numeric value with unit, denominator and period where given, population, method or evidence type, and limitation. Preserve original wording where a short quotation is needed and allowed; paraphrase otherwise. Separate reported findings from the document author's interpretations and recommendations. Include appendix data only when supplied. Note conflicting figures with both locations and mark unresolved. Extract the document's recommendations, stated owners and dates, and theory-of-change links only when those appear; do not create them in an extraction. Mark missing limitations as not discussed by the source, rather than inventing a list. End with a compact list of evidence gaps relevant to the stated focus. If only a document excerpt is available, bound the register to that excerpt.
