# Start: Findings to management actions

Use this skill to make evaluation findings actionable through a traceable response and action matrix. It separates evidence, management decisions, and proposed actions. Suggested owners and dates are not commitments.

**Intake:** attach findings and relevant responses, or name a folder. For a folder, ask the assistant to list files before selecting them. It must state the intake mode.

**Starter:**
