# Fictional input: monitoring data flow and known risks

## Program

Clearhaven Rural Water is an invented community water program. This file and all figures are fictional and supplied only as an example.

## Purpose/use

The program manager and funder use monthly monitoring data to prioritize repairs and report whether registered water points are functioning.

## Indicator A

Percent of registered water points functioning at the monthly visit. The May report lists 84 functioning points out of 110 registered points. A draft field guide defines “functioning” as “provides water,” but the site logs sometimes use a flow test and sometimes absence of an open repair ticket. The definition has not been approved.

## Indicator B

Number of monthly water-quality samples with a recorded result and sample identifier. The May summary reports 14. The laboratory export contains two results whose sample identifiers do not appear in the summary. No sample-level acceptance threshold was supplied.

## Data flow

Four area technicians record site observations and sample IDs in local logs. An area supervisor enters monthly totals into a shared workbook. The monitoring officer checks and copies totals into the dashboard and monthly report. Versions and submission dates are available in the workbook, but no change log was supplied.

## Known risks

The monitoring officer flagged three repeated water-point IDs in the May asset list. The report is used for repair prioritization and funder reporting. No earlier DQA or action record was supplied.

## Access and constraints

The monitoring officer can provide the workbook, dashboard export, site logs, asset list, and laboratory export for desk review. The team has two staff workdays for this assessment. No approval for household contact, identifiable household records, site visits, or independent laboratory retesting was supplied. Site-visit safety arrangements are unknown.

## Standards and tolerances

No approved discrepancy tolerance, acceptance threshold, or completeness rule was supplied.
